FAQ

Change my personal details

If you change your name, address or telephone number, please let our receptionist know.

Are you travelling?

• Are you going abroad?
• Have you checked whether you need vaccines?
• Do you have an EHIC card?
• If not see our nurses!! at least six weeks in advance!!!

Sick certificate

• By law patients are entitled to self certificate for seven days on SC2 form from the employer
• The doctor cannot backdate certificates.
• Should you need an extension- you should book an appointment to see the doctor before it runs out.
Zero tolerance

We support the NHS policy on zero tolerance. Anyone who abuses our staff be it verbally, physically or in any threatening manner will risk removal from the practice list.

Patient Group

• We are looking for a small group of patients who would like to join a patient group.
• Have your say in your healthcare.

Contact the receptionists if you would like to join.

Housebound

Special exceptions can be made for patient in this category, please contact the surgery.

Home visits

The doctors will do home visits only in genuine serious emergencies.
The doctor will normally discuss the home visit, so that the doctor can understand the nature of the emergency they have to deal with.

Transport

Patients may qualify for transport for their appointments in the hospital, please contact ERS medical and patients ring them direct to order the transport 0333 2404083

Choose & book

Referrals will be made through the Central Referral System and Choose & Book. You will receive a letter with full instructions giving you a choice of hospital and / or clinic. You will then be able to either telephone the appointments line or go online where you can make your appointment at a time suitable to you.

Disabled access

The surgery has a side access with a ramp to allow for wheelchairs.


Complaints

Our aim is to give you the highest possible standard of care and to try to deal swiftly and thoroughly with any problems that may occur.
We operate a practice procedure as part of the NHS system for dealing with complaints.
If you would like to make a written complaint regarding the surgery or the services we offer please contact the Practice Manager in writing .
Every effort will be made to answer your concerns as soon as possible.
Any compliments are most welcomed.

Access to medical records

The practice is registered and complies with the Data Protection Act 1998 and the General Data Protection Regulation (GDPR) May 2018.
Any request for access to medical records by a patient, patient’s representative or outside body will be dealt with in accordance with the Act.
